BANNER The user guide provides some examples of a banner on the dialogs, but I cannot seem to find the actual XML that is used. It is also not clear to me if I can set a tall header on all dialogs and add a banner, and what is the recommended pixel dimensions?
BORDER When I set the UIApp color to white (my ICO file is a white background) and the dialog looks hooky without a border around the title bar. Is it possible to configure a border to be around the entire dialog, and can I set the color of the border, thickness/style.

There is an example for this in the documentation: Example 2 shows this and gives the XML snippet:
<Action Type="Info" Name="myInfo" Title="Welcome" Image="my.png" ShowCancel="True">
The Image attribute is only valid on Info and ErrorInfo actions.
As for the border, have you tried using the default, rounded dialogs and not the flat dialogs?

Thank you Jason, I must have missed the Image attribute. What is the recommended/supported dimensions for the banner? Some feature asks: 1 - Allow image on any dialog, but if that is a lot of work, at least the User Auth dialog. That is going to be both my welcome and my auth screen. Really trying to minimize the clicks and the Next>Next>Next kind of flow. 2 - Border - No the flat mode is worse. It would be good if I could specify a border all the way around the dialog. Attached some comparison images (without the logo icon)

This is done (borders on flat dialogs) and will be included in 2.11.0.0 which I should be releasing this weekend.
